As a travel nurse, I have to apply for an exemption for each facility I work. Had to apply for 5 in past two years and each one was approved, including Florida, Pennsylvania and even now New York. The facility should have a form they want filled out, one for medical and one for religious. A medical will need a provider to sign. I have a very strong religious objection so was able to go the religious exemption route. If you have a strong religious opposition, I would go that route. Some facilities/states allow for personal belief objections. It should state on form but be careful not to go too much into how bad the vaccines are, these people still believe in them. I went mainly with my unbelief in unnatural immunity. Good luck.

I recently worked as a travel nurse at Reading Hospital, part of Tower Health System. They accepted my religious exemption for the COVID vaccine. They have a nursing school so may be worth inquiring about.
